A large number of studies show that the risk of sudden death is higher when babies are prone than when they lie on their backs or sides. Some researchers also hypothesize that when babies sleep on their stomachs, they press on the jawbone and will narrow the airway and interfere with breathing.

Another hypothesis is that when a baby sleeps on his or her back, it may increase the risk of breathing again, especially if the child sleeps on a soft mattress or has stuffed toys, or a pillow near his face. In that situation, the soft surface of these objects can create a trap to trap the exhaled air of the child. So the baby will breathe back their exhaled air, so the level of oxygen in the body will decrease and carbon dioxide will accumulate. Finally, hypoxia can contribute to sudden death syndrome in children.
